About IRS Tax Forms

You will receive forms in the mail either from Calamos Investments or your broker (depending on where your account is held) to help you prepare your taxes. Look below for information about certain forms pertinent to your mutual fund investment.

Forms, publications and instructions are also available for download from the IRS Web site.

IRS Forms
1099-B
 Proceeds from Broker and Barter Exchange Transactions
IRS Form 1099-B reports proceeds from redemptions and exchanges of mutual fund shares including date of each sale, gross proceeds, income tax withheld, transaction description, number of shares sold and share price of each transaction. Although not a tax form (and not reported to the IRS), you will receive the Average Cost Accounting Statement with your 1099-B. Retirement plan accounts or accounts that have had a change in ownership or control will not receive this additional statement. This statement includes the trade date, number of shares redeemed, redemption price, redemption proceeds, average cost basis and gain (or loss) on each transaction. This statement may help you to report gains and losses to the Internal Revenue Service (IRS) on the sales of mutual fund shares in the current tax year.
1099-DIV
 Dividends and Distributions
IRS Form 1099-DIV reports income and capital gains distributions (cash or reinvest), as well as qualified dividends, nontaxable distributions, foreign tax credit and withholding, from your taxable mutual fund accounts.
1099-Q
 Payments from Qualified Educational Programs (Under Sections 529 and 530)
IRS Form 1099-Q reports all distribution activities from a qualified educational program. These can include routine distributions, rollovers and trustee-to-trustee transfers, as well as return of excess contributions.
1099-R
 Distributions from Pensions, Annuities, Retirement or Profit-Sharing Plans, IRAs, Insurance Contracts, etc.
IRS Form 1099-R reports distributions from retirement plans, profit-sharing plans or insurance contracts, as well as IRA recharacterizations. It includes the gross distribution amount, taxable amount and withholding information.
5498
 IRA Contributions Information
IRS Form 5498 reports retirement account contributions, rollovers, Roth IRA conversions, recharacterized contributions, and the fair market value of the account. This includes Traditional IRAs, SEP, Simple and Roth IRAs.
5498-ESA
 Coverdell Education Savings Account Contributions Information
IRS Form 5498-ESA reports all Coverdell ESA (CESA) contributions and rollovers.
1042-S
 Foreign Person's U.S. Source Income Subject to Withholding
IRS Form 1042-S reports a nonresident alien's U.S. taxable income, including distributions from Calamos Investments and federal tax withholding.
W-9
 Request for Taxpayer Identification Number and Certification
IRS Form W-9 requests the taxpayer's identification number and certification.
